Purpose of the Program
The Well Child Health Assessment Program assesses children for normal development and growth. Children 0-18 years of age are given an examination that may include:
  • Complete physical examination
  • Developmental screen
  • Education and information pertinent to your child's age and assessment findings
  • Fluoride Varnish
  • Growth assessment (height and weight)
  • Gynecological exam deferred and done as needed through the Family Planning / Well Woman Programs
  • Hearing screening
  • Lab Testing as indicated
  • Immunization status review and immunizations administered as needed
  • Lead screening
  • Referral to health care provider and other resources as indicated by exam
  • Vision screening

There is a sliding fee scale available based your household gross income and your household size.

Enrollment / Appointment Process
Call 785-457-3719 for appointment. Complete the papers that are mailed to you. Come with your child to the appointment that will last about 1.5 hours. 

***Health Department staff are not approved by Kansas State High School Activity Association for Sports Physicals.
